Foria provides surgery assessments in Alberta, British Columbia, and Ontario. This question only applies to patients in Alberta and Ontario. In British Columbia, only one assessment is required for surgery, so there is no second assessor needed. For Alberta and Ontario patients, we can provide one or both of your assessments for bottom surgery.
Alberta
Only the physicians at Foria and the gender clinics in Alberta can complete surgical assessments and submit the out-of-province prior approval funding applications.
If you would like us to provide both of your assessments, we can prepare and submit your paperwork to Alberta Health. The outcome of each application will be sent directly to us, and we can update you when this has arrived.
If you would like us to provide the first of two assessments, the gender clinic must send us your second assessor paperwork. This is because the first assessor must be the one to submit your application to Alberta Health. After your second assessment is complete, your assessor needs to fax us the paperwork at +1 437 317 9843.
If you would like us to provide the second of two assessments, we will send the paperwork to the gender clinic, who will then submit your funding application to Alberta Health. The outcome of your application will be sent to the gender clinic.
Ontario
If you would like us to provide both of your assessments, we can prepare and submit your paperwork to the Ministry of Health. The outcome of each application will be sent directly to us, and we can update you when this has arrived. If you are getting two assessments with us, you will need to book two separate assessments with different Foria clinicians.
If you would like us to provide the first of two assessments, your second assessor must send us some paperwork. This is because the first assessor must be the one to submit your application to the Ministry of Health. After your second assessment is complete, your assessor needs to send us Page 6 of the Prior Approval Form and their second assessment consultation notes. They can fax these to us at +1 437-317-9843.
If you would like us to provide the second of two assessments, we will send Page 6 of the Prior Approval Form and your second assessment consultation notes to your first assessor. They can then submit your application, and they will be notified of the outcome. After your assessment with us, we’ll send you a questionnaire where you can give us your first assessor’s information.
